How Menopause Medicine Should Work.
Every consult, every lab, every adjustment. Here’s exactly what care in the Orvi network looks like — from the first 45-minute intake to the quarterly check-ins that follow.
What Gets Measured, and Why.
A menopause workup requires specific hormones, not a standard chemistry panel. Every Orvi patient’s baseline includes these markers — and a physician personally reads every result.
Estradiol (E2)
The primary active estrogen. Baseline for any estrogen therapy decision.
Progesterone
Critical for sleep, mood, and endometrial safety if estrogen therapy is indicated.
FSH & LH
Elevated FSH is one of the clearer biochemical signals of the transition.
Total & Free Testosterone
Often low in midlife women; under-measured in primary care but frequently relevant.
DHEA-S
Adrenal precursor hormone that declines with age. Context for the full picture.
Thyroid (TSH, Free T3/T4)
Thyroid issues mimic and compound menopause symptoms. Every panel screens.
Metabolic & Lipid Panel
Glucose, A1C, cholesterol, triglycerides. Safety plus adjacent-issue detection.
Vitamin D & CBC
Vitamin D, basic complete blood count, liver function. Standard safety and context.
From Application to Steady Care.
Day 0 — Apply
Short, private application. Read personally by a member of our team within 24 hours. You hear back either way — accepted, not a fit, or a follow-up question before we decide.
Days 1–2 — Onboarding fee
If accepted, you pay the $297 one-time onboarding fee. This covers every piece of what comes next: intake, lab coordination, provider consult, prescription setup, and the 30-day follow-up.
Days 3–7 — Intake consult
A 45-minute video consult with a menopause-trained physician licensed in your state. Unhurried. History, symptoms, goals, and the shape of the plan going forward.
Days 3–14 — Labs
A lab order is issued at your intake. You visit a walk-in LabCorp or Quest near you. Results typically return within 3–5 business days.
Days 14–21 — Protocol consult
A second physician consult to review your labs together and design the protocol. For most patients this includes bioidentical estrogen, progesterone, and/or testosterone; for some it involves non-hormonal routes. Nothing is templated.
Days 21–28 — Prescription fills and ships
Your prescription is filled by an FDA-regulated U.S. pharmacy and shipped to you. Packaging is discreet. Clear instructions included.
Day 50 — 30-day follow-up
A structured check-in with your physician. Thirty days is long enough to see early signals and short enough to catch and correct anything that isn’t working. This is included in the onboarding fee.
Month 2 onward — ongoing care
From month two, the $147/monthly covers prescription fulfillment, refills, and a quarterly physician check-in. You stay with the same physician. Adjustments are made as needed, based on how you’re actually doing.

A 45-Minute Intake
A real conversation — not a six-minute visit — with a menopause-trained physician licensed in your state. Unhurried by design.
Comprehensive Hormonal Panel
Estrogen, progesterone, testosterone, FSH, LH, thyroid, metabolic, vitamin D, and safety markers. Collected at walk-in labs near you.
Protocol Consult
A second physician consult once labs return to review together and design your protocol based on YOUR numbers.
Bioidentical Hormone Therapy
When indicated: FDA-approved bioidentical estrogen and progesterone. Testosterone, when prescribed for women, is used off-label based on current clinical evidence. Everything is prescribed to your labs and goals, not a template.
Direct Physician Messaging
Secure messaging with the physician for questions between consults. Real clinical judgment, not chatbots.
30-Day Follow-Up
A structured check-in at the 30-day mark to review how the protocol is landing and make early adjustments if needed.
Quarterly Check-Ins
Ongoing structured physician visits every three months. Your protocol evolves with you; nothing is "set and forget."
